Did you know that improving cough function may also improve swallowing safety?

In a landmark study by Pitts et al. (2009), individuals with Parkinson’s disease at risk for aspiration completed 4 weeks of Expiratory Muscle Strength Training (EMST) resulting in improved cough effectiveness and significantly reduced penetration-aspiration scores.

These findings highlight that cough and swallowing are both critical components of airway protection, and deficits in one system can directly impact the other.

Expiratory muscle strength training (EMST) is understudied in pediatric asthma. Most research focuses on inspiratory muscle training, leaving a gap in how we approach the full respiratory picture in kids.

This study assigned 30 children with asthma, ages 8 to 18, to either EMT at 30% of maximal expiratory pressure plus chest physiotherapy, or a sham protocol, for 8 weeks.

The EMT group showed significant improvements in FVC, FEV₁, peak expiratory flow, maximal expiratory pressure, peak cough flow, quadriceps strength, 6-minute walk distance, and Asthma Control Test scores. The sham group improved in some measures, but consistently less.

EMT appears to be a safe, practical adjunct to pediatric asthma management, with benefits that extend beyond pulmonary function.

Children who undergo kidney transplantation may continue to experience respiratory muscle weakness and reduced functional capacity well into the post-transplant period, a clinical reality that rehabilitation protocols don’t always address.

This RCT examined the effects of an 8-week combined IMT + EMT program integrated with chest physiotherapy in pediatric kidney transplant recipients ages 8 to 18. Compared to chest physiotherapy alone, the combined training group showed significantly greater improvements in pulmonary function, respiratory muscle strength, and 6-minute walk distance.

The findings support respiratory muscle training as a structured component of post-transplant rehabilitation in this population.

A 2026 systematic review and meta-analysis in Frontiers in Physiology examined the effects of respiratory muscle training on competitive swimmers — looking at respiratory muscle function, pulmonary function, and athletic performance across 14 studies.
RMT significantly improved inspiratory muscle strength and dynamic lung function parameters, including FEV1 and FIV1. Aerobic swimming performance also improved. Effects on sprint and middle-distance times were not significant, which the authors attribute in part to high inter-study heterogeneity.
Combined RMT — inspiratory and expiratory together — showed stronger outcomes than inspiratory training alone.
Swimming places unique demands on the respiratory system. The research here suggests that training the muscles responsible for meeting those demands has measurable value, particularly for aerobic capacity and respiratory function. Worth following as the evidence base continues to develop.​​​​​​​​​​​​​​​​

The evidence continues to support the potential benefits of Inspiratory Muscle Training (IMT) across rehabilitation, mobility, balance, surgical recovery, chronic disease management, and healthy aging.

This perspective paper (Sheraz et al., 2026) highlights how IMT can improve inspiratory strength, reduce dyspnea, support exercise tolerance, improve postural stability, and enhance quality of life.

IMT sits at the intersection of respiratory care, rehabilitation, prevention, and functional health. It is portable, scalable, relatively low cost, and increasingly relevant as healthcare looks for accessible interventions that support long-term function and quality of life.

There is still important work ahead around protocol standardization and long-term outcomes, but the growing body of evidence is encouraging to see.

At Aspire Respiratory Products, we are encouraged to see continued research exploring the role of inspiratory muscle training in cardiovascular health.

This systematic review and meta-analysis (Gurovich et al., 2026) found that inspiratory muscle training (IMT) may help improve inspiratory muscle strength while also contributing to modest reductions in diastolic blood pressure and pulse pressure in adults with elevated or high blood pressure.

The authors also highlighted strong adherence rates, good safety outcomes, and potential vascular benefits, reinforcing IMT’s promise as a practical, home-based adjunct within cardiopulmonary rehabilitation.

As always, more long-term research is needed, but it is exciting to see the growing evidence supporting respiratory muscle training across healthcare settings.

The conversation around respiratory muscle training keeps expanding.

A 2026 mini review in Frontiers in Rehabilitation Sciences examined RMT’s potential role in cognitive function across older adults, post-COVID patients, COPD, and OSA populations. The findings are preliminary, only one study reported statistically significant between-group differences in specific cognitive domains, but the direction is worth watching.
