If you require copies of dispensing records or specific prescriptions you may contact the custodian of records. These records are for information only and unfortunately cannot be transferred to another pharmacy. If you require a prescription at a new pharmacy please contact your doctor for a new prescription.

To request dispensing records please contact either by phone (leave a message) or email and provide name, DOB, what records are being requested (for example; last 6 months, last year, specific year, or specific date range) and where to have them mailed to you.

Update 1/10/24
Due to unforeseen circumstances, Broadway pharmacy has decided they cannot accept or accommodate our prescription files. I am working on trying to get them to another phar, however the situation may be they will just have to be with a “custodian of records” which means to get prescriptions filled patients will need to have their doctors send their prescriptions directly to the pharmacy of their choice. I apologize for any inconvenience this causes.

It is with great regret that we announce the closure of Reedsport Pharmacy.

The last day we will be open for prescriptions will be
December 29, 2023.

From now until that time we will continue to fill prescriptions that are available or in stock. For future prescriptions, have either your mail order pharmacy or a pharmacy of your choice call us and we will transfer any active prescriptions and their refills.

After December 29 prescription files will be transferred to Broadway Pharmacy at
2790 Broadway Avenue, North Bend, OR 97459.
