FOR COMPLIANCE: Online Faculty Evaluation System (OFES)

Instructors' evaluation is an important factor to improve the quality of learning. Thus, the university is implementing the Online Faculty Evaluation System (OFES).

This assessment provides a platform to recognize the strong teaching abilities of your instructors. It also helps to identify ways to improve the effectiveness of quality teaching.

Upon using this tool, please be reminded to assess your instructors objectively. Such evaluation assesses the teaching force’s performance for compliance to standards and effect improvements if necessary. Accepting feedbacks could make an immense change to one’s performance and influences others for development.

𝐆𝐞𝐧𝐞𝐫𝐚𝐥 𝐆𝐮𝐢𝐝𝐞𝐥𝐢𝐧𝐞𝐬
1. The search is open to all interested students and alumni of Cagayan State University.
2. Only one (1) entry per participant will be accepted.
3. All entries must be in digital with the following attachments:
a. Soft copy (editable file from Adobe Photoshop with 300
dpi/resolution).
b. Soft copy in JPEG and PNG file.
c. The rationale of the logo and its details, concept, elements
and designs not exceeding 500 words.
4. All entries must be submitted on or before November 22, 2021. It shall be named using the format:
[Surname]_[First Name]_[Campus]_Logo Contest
(Subject)
(Example: Dela Cruz_ Juan_Carig_Logo Contest)
5. All entries shall be submitted to 𝐦𝐚𝐫𝐢𝐚𝐛𝐞𝐫𝐧𝐚𝐝𝐞𝐭𝐭𝐞𝐦𝐚𝐫𝐜𝐨𝐬𝟐𝟖@𝐠𝐦𝐚𝐢𝐥.𝐜𝐨𝐦.
6. Submitted work shall be the original work of the participant. The participant shall be held accountable for any issues that may question the originality and authenticity of the design.
7. Two(2) entries will be chosen to be submitted to our Campus ARPS and the best entry will be submitted to the University ARPS. The winner will be announced on December 7, 2021 (university level) and shall be awarded with a cash price.
8. All submitted entries shall become the property of the Office of the Federated Alumni Associations of CSU Inc.
9. The Federation reserves the right to modify the winning logo.
10.All entries will be judged based on the following criteria:
Concept/Relevance (40%) - The logo must represent the Federation, its objectives and purposes.
Originality (30%)- The logo must have a fresh and original idea
Creativity & Impact (30%) - The logo must be appealing and must have harmony and agreement with the design.
11.The winning entry will receive a cash prize of P5,000.00 and a certificate. The second-place winner will receive P3,000.00 and a certificate, while the third-place winner will receive P2,000.00 and a certificate. (University level)
