Iron is the quiet one. If you feel worse the week after your period instead of better, that is depletion, not hormones. Ask for ferritin, not just hemoglobin.

If the pattern changes cycle to cycle, look at whether you are ovulating at all. No ovulation means no progesterone phase to feel. Stress, undereating, heavy training, and PCOS all live here. Perimenopause brings erratic estrogen rather than low estrogen, and it often starts while periods still look regular.

In Chinese medicine the swinging itself is liver qi stagnation. The crash is spleen qi. The flat, depleted version is blood deficiency.

Two cycles of basal temperature tracking alongside your energy will tell you if you are ovulating and how long your luteal phase runs. That cuts the list in half.

Estrogen climbs through the late follicular phase into ovulation, and it takes verbal fluency, verbal memory, and social confidence up with it. You access the words faster and you stay regulated while you use them. This is the window for the raise, the boundary, the conversation you have been putting off.

Early luteal, the few days right after ovulation, is steady and good for follow through. Logistics, plans, the practical version of a conversation you already had.

Late luteal is the one that gets women labeled. The week before your period, estrogen and progesterone both fall and serotonin drops with them. What most people miss is that the noticing is not distortion. The things you see that week are usually real. What changes is your capacity to hold them quietly.

So use it as intake, not delivery. Write down what you notice. Do not send it. Read it again on day 9 and you will know which parts were signal.

Then it falls in the second half of luteal, and the lowest point is your period.

If your energy doesn’t follow that pattern, look at iron, at your luteal length, or at whether you’re ovulating at all.

Flow is the part most women dismiss and it carries the most information. Heavier bleeding in the forties is common but common is not the same as fine. It often means cycles where estrogen builds the lining without progesterone to organize the shedding, and it can also point to fibroids or adenomyosis, both of which peak in that decade. The cost accumulates quietly through iron. Ferritin drops long before hemoglobin does, so the fatigue, hair shedding, and cold hands show up while standard labs still read normal.

In the medicine I practice, flow reads as a direct report on blood and qi. Scant and pale points toward blood deficiency, dark with clots toward stasis, heavy and bright toward heat pushing blood out of the vessels. Same information the labs give, arriving earlier and for free.

What actually warrants a call:

• Soaking through a pad or tampon every hour for several hours
• Clots consistently larger than a quarter
• Bleeding past seven days
• Bleeding between cycles or after s*x
• Any bleeding after twelve months without a period

Tracking also makes you a better historian. When you sit down with a provider at 44, the questions are how long, how heavy, how often, and how much has changed. Most women guess. Guessing is how a real problem gets filed under normal aging for three years.

Once you have that baseline, the leverage sits earlier than people expect.

The complaints that show up in the luteal phase and the days before bleeding are usually late signals of what happened in the follicular phase and during the period itself.

In TCM terms that’s where blood gets depleted and qi starts to stagnate. Address the upstream inputs (real sleep, adequate iron and protein, not undereating while you’re bleeding, moving less intensely in the days you’re actually losing blood) and a lot of the downstream symptoms quiet without being treated directly.

There isn’t a “single hack.”

There’s a baseline you either have or don’t, and a habit of matching your inputs to the phase you’re actually in rather than the phase a generic app says you’re in.
