Dear Valued Customers,

In the near future the FDA is going to enforce a ban on all va**ng products that have not completed a market approval process. One part of the requirements for approval is information on how and why consumers are using va**ng products. By filling out this survey you are helping us collect that information to submit to the FDA. The survey is 13 questions and will take just a couple minutes.

All our store locations have this survey available for customers to fill out. There is also a copy attached here which you can print out and either return to one of our store locations or send pictures to one of our pages.
